Resume
Active clinical license for your discipline
Malpractice insurance with at least $1,000,000 each claim and $3,000,000 aggregate coverage
Current CPR card
Physical exam stating no work restrictions, dated within the last 12 months
TB test results dated within the last 12 months
Auto insurance
EIN confirmation
COVID vaccine record
